Ultimate Vault Hunter Mode ramps up the difficulty and the rewards as it serves as the endgame in Borderlands 4. While totally optional, it does require some work to unlock in the first place, not to mention grinding through its content for even higher ranks.

Unlock Ultimate Vault Hunter Mode
Completion of the campaign is the absolute first step, followed by a series of quests which you will pick up by speaking with Lilith at the bar in Moxxi’s Bottoms Up! station located in the Ruined Sumplands of Carcadia Burn.
After the first Ultimate Vault Hunter: Wildcard mission from her, you will automatically activate the lowest level of Ultimate Vault Hunter Mode beginning at level one.
Raising or lowering the difficulty can be accomplished at will via Session Settings in the Main Menu.
Subsequent levels are unlocked in the same manner: a series of four Ultimate Vault Hunter challenges, capped off with a new Wildcard mission, but you must play them on the highest possible preceding difficulty unlocked at the time.
The final Ultimate Vault Hunter Mode difficulty level is rank five.
Ultimate Vault Hunter Mode Mechanics
Enemy health, damage received, and XP will scale up with each rank. Cash, Eridium, and general loot drop quality will also increase significantly.
